Instructions and data collection tools for collecting health system cost data designed by the Department of Community Medicine and School of Public Health, PGIMER, Chandigarh for Cost of health services in India (CHSI) Study data collection are presented here. The data collection tool is based on international best practices using an economic costing methodology and a bottom-up costing approach. A separate tool has been developed for each level of health facility; i.e., secondary and tertiary care hospitals are further segregated at public and private facilities. The data collection tools also take into consideration the design of health facilities in terms of variation in the type and quantity of service delivery and resources used.
